Bupa Home Healthcare is currently recruiting for a Community Based Nurse specialist in the delivery of Intravenous Therapies. This position is community based covering the Essex and East Hertfordshire border.
 
You will work closely with patients and their families to ensure a high standard of care is delivered. You will plan patient centred care according to their needs. You will also liaise with a wider multidisciplinary team and when required assist with discharge planning to ensure a smooth transition from hospital to home. 
 
We provide a range of IV Therapies in the Community including IV antibiotics, TPN/HPN, and growth hormones. Previous IV, TPN and PICC line experience is essential.

           Assess, plan and implement ongoing care to patients. 
           Providing Intravenous care including Antibiotics, HPN/TPN, growth hormones and MS patients.
           Communicate with the patient and keep the family well informed
           Document all aspects of care delivered

A Service based in the Berkshire area requires tier 3 substance misuse team leader to join their busy team. Experience in substance misuse nursing is essential, experience in management would be of benefit.

On a day to day basis the successful candidate will assess, plan, implement and evaluate structured treatment programs for individual clients in accordance with agreed quality practices and procedures. Additionally the successful candidate will manage a team of substance misuse practitioners and nurses on a daily basis and provide advice and support for staff who are directly involved with treatment work.This role provides a good opportunity to gain valid experience and training; there is also a possibility of contract extensions and permanent contracts.

You will also be responsible for acting up in the team managers absence and attending relevant inter-agency meetings about the ongoing delivery of the IDTS program.

The working hours of this role are full time and cover 7 days per week. The ongoing development of the service and the role itself presents the opportunity for the right candidate to develop a range of mental health skills working in a challenging criminal justice environment. As a Manager you will be responsible for the day to day running of the home.
