I have a fantastic position that has come available for a Case Coordinator to work within an Adults/Older Persons Strategic Team. You will be working with palliative care service users and those who have been approved for continual health care. The successful candidate will be responsible for the arrangement and co-ordination of plans for problematic and complex caseloads. They will research needs and availability of relevant community resources within Dorset to ensure that the needs of both client and family are met. Experience of working with clients with physical or learning disabilities a benefit. Any experience of working with clients with neuro issues or autism is benefit however there is the opportunity to gain experience with this client group.
You will need to be either a Registered Nurse, Registered Mental Health Nurse or Qualified Social Worker with a current GSCC registration to be accepted for this role.
You will be joining a small & friendly team with the potential of being offered a permanent position after a short probation period.

An experienced Clinical & Palliative Nurse specialist is required for our client based in London (N11 area). Working for a large nursing home. You must have palliative and end of life care experience as you will be implementing an intervention programe and will need to be a palliative care specialist. You will be required to be an excellent Practitioner, trainer and mentor and you will be working with staff, nurses and care managers to support, improve and upgrade nursing practices and educate staff to raise the level of nursing care throughout the Home.
You will be reporting to the Registered Manager but will also deputise for him/her.
You will be responsible for taking the lead in the End of Life Care in Advanced Dementia (ELCAD) project developing and implementing good end of life practice including Gold Standard Framework.
To develop nursing staff and members of the multidisciplinary team, providing support, advice, formal and informal education, relfective practice and clinical supervision.
You will work closely with the unit care managers and oversee and monitor the nursing practice of around 30 day and night nurses.
You must be:
First Level Registered Nurse (RGN)
Recognised qualification in End of Life/Palliative Care such as ENB 237 or 285/931 or similar
Teaching qualification such as ENB 998 or similar
ENB N11 Caring for People with Dementia or similar
Diploma in Palliative Care would be an advantage
Health Related first degree or equivalent would be an advantage
