Job Summary
We’re looking for Nurse Care Coordinators who understand the principles of health promotion and the disease process to apply these within practice. You’ll assess the care required by clients and take into consideration the long-term physical, psychological and social impact a specific diagnosis and treatment can have upon a client’s rehabilitation. In this role you’ll have the ability to recognise the need to adapt education and training to meet individual clients needs.
Key Responsibilities
•You will be responsible for ensuring that you work effectively with your team and customers
•You’ll ensure that you deliver a quality and effective service
•Prioritise requirements of role and ensure that requirements are met each shift
•Manage the care delivery for a shift. Ensure that the boundaries of the role of Nurse Care Coordinator and client/family are maintained
Specific Skills
•RGN/RSCN qualification with post qualification experience (essential)
•NHS or community background (desirable)
•ENB 998 or equivalent (desirable)
•IV experience (essential)
•Cannulation/venepuncture experience(desirable)
•Ability to communicate with client, client’s family and a range of health and care professionals
•Must be flexible to travel to clients homes
•Experience in home healthcare is not essential as we will provide you with excellent education, training and development and other rewards which are everything you would expect from a pioneer in home healthcare.
