You will work closely with patients and their families to ensure a high standard of care is delivered. You will plan patient centred care according to their needs. You will also liaise with a wider multidisciplinary team and when required assist with discharge planning to ensure a smooth transition from hospital to home. 
 
We provide a range of IV Therapies in the Community including IV antibiotics, TPN/HPN, and growth hormones. Previous IV, TPN and PICC line experience is essential.

           Assess, plan and implement ongoing care to patients. 
           Providing Intravenous care including Antibiotics, HPN/TPN, growth hormones and MS patients.

We are looking for a Community Nurse Manager who will be working closely with paediatric patients and their families. You will ensure that a high standard of care is delivered by your nursing team which consists of Community Nurses and Clinical Support Workers. 
 
You will plan patient centred care according to their needs. Patients have complex care requirements including tracheotomy and ventilators. Previous experience of these clinical skills is preferable.
